By voluntarily choosing to allow Tribe of 1, LLC to provide services, the undersigned understands, acknowledges and agrees that:
The services provided by Tribe of 1, LLC are not a substitute, nor are they intended to replace licensed professional medical care. The services provided by Tribe of 1, LLC are not intended as diagnosis, prescription, treatment, cure or prevention for any disease, disorder, injury, physical or mental.
Tribe of 1, LLC makes no claim as to any medical, physiological, or psychological purpose, effect or outcome in connection with our services.
The undersigned’s participation is of his/her own volition and the undersigned warrants and represents to Tribe of 1, LLC that he/she does not have any mental, physical, or emotional conditions which would hinder participation and/or cause harm to the undersigned.
Tribe of 1, LLC and its employees retain the right to refuse service to any person for any reason at any time.
In permitting the undersigned to undertake participation in the services that Tribe of 1, LLC offers, Tribe of 1, LLC, its owners, affiliates and employees are relying upon the undersigned’s assumption of the risks associated with participation and the undersigned’s willingness to release them from any and all claims or damages arising therefrom.
RESCHEDULING, CANCELLATION, AND NO-SHOW POLICY
We are serious about what we do and won't waste time on anyone that isn't serious about their Spiritual Progression. If you are asking us for assistance, then help yourself first by honoring your commitment.
RESCHEDULING/CANCELLING/NO-SHOWS: If you must reschedule/cancel your Initial Consultation, Spiritual Guidance, or Shamanic Work appointment, we require AT LEAST 48hrs advance notice through our Contact page or by emailing us@tribeof1.org. If you are running more than 5 minutes late for a scheduled appointment, this will be considered a no-show.
IF YOU GIVE LESS THAN 48HRS NOTICE OR ARE A NO-SHOW, YOU WILL BE PERMANENTLY DROPPED FROM OUR CLIENT LIST AND INELIGIBLE FOR ANY FUTURE SERVICES WITH US.
RECORDING DEVICES: Unless permission is granted by Tribe of 1, LLC in writing prior to, the use of recording devices is strictly forbidden during Zoom Meetings, Spiritual Guidance, Initial Consultations, Follow-Ups, healing sessions, shamanic sessions, Group Classes, workshops, conferences, keynote speeches, events, or any other Tribe of 1, LLC programs or at such programs sponsored or presented by Tribe of 1, LLC. Any person violating this will be permanently dropped from our client list and ineligible for any future consultations/work with us. No exceptions.
PRIVACY NOTICE
Tribe of 1, LLC respects your privacy. The purpose for requesting your contact information is to assist us in providing you with up-to-date information regarding the services purchased and responding to your requests and/or questions. Tribe of 1, LLC may employ a third party to distribute promotions including the Tribe of 1 email newsletter. These parties have agreed to hold this information in confidence and not use it for any purpose except to carry out the requested service(s).
CHANGES IN TERMS Tribe of 1, LLC reserves the right to change the TERMS or policies at any time and to notify you by posting an updated version of the TERMS on our website (www.tribeo1.org). I, the undersigned, also have a right at any time to request a copy of the latest Terms and Conditions and Privacy Policy. These Terms and Conditions and Privacy Policy were posted on October 31, 2022.
NOW THEREFORE, for good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the undersigned agrees to accept and assume any and all risks to his/her health and well-being, including any risk of bodily injury or harm, that may arise from participation whether such risks have been disclosed or not, and does hereby fully release and forever discharge Tribe of 1, LLC, its owners, successors, affiliates, assigns, directors, officers, employees and agents from any and all actions, causes of action, suits, liabilities, losses, damages, claims or expenses whatsoever, in law or in equity, for negligence, battery or otherwise, which the undersigned, their heirs, executors, administrators, and assigns have now or may hereafter have arising in any way out of the undersigned’s Participation, including, but not limited to, the undersigned’s participation in physical, psychological or emotional activities which may or may not lead to healing emotionally, mentally, physically or spiritually at any of our events, workshops, speaking engagements or private healing and/or Shamanic sessions.
The undersigned states that he/she has read this acknowledgement and release, that he/she understands the terms contained in it and that he/she has freely and voluntarily executed the same and intends to be bounds by its terms.
